 What should I bring to my appointment?

  • Driver’s license (or another form of government-issued photo ID)

  • Insurance card

  • Competed patient information form

  • All x-rays, MRIs, CAT scans on CD-ROM performed in the past 5 years, if any.

What insurance plans does PainCare P.C. accept?*

  • Blue Cross

  • Aetna

  • Cigna

  • United Healthcare

  • Medicare

  • Motor vehicle insurance

  • Workman’s Compensation (with referrals only)

  • Private Liens

  • We do not participate in Medicaid. 

Where your insurance plan requires referrals for office visits or consultations, you must obtain the referral before your visit to our office.   Patients are required to pay deductibles and co-pays required by your health insurance.

We see patients with insurance plans featuring out-of-network benefits.

*Some insurance plans feature “Narrow” networks where only one or two physicians are allowed to see patients in a particular specialty.  While saving money for the insurance company, this practice restricts access to high quality care.

We offer fee-for-service cash payment options for second opinions, including patients covered by Kaiser-Permanente.

Certain procedures are not covered or not fully covered by insurance plans or Medicare. Our office will attempt to advise you accordingly.
